Among this issue’s excellent reviews, Kautzner and Peichl examine ventricular arrhythmias originating from the papillary muscle of the left or right ventricle. The authors acknowledge that whilst radiofrequency ablation has the potential to cure these arrhythmias, it presents certain challenges, notably catheter instability. The authors discuss cryoablation – which ensures catheter tip stability – as a viable alternative in cases of the failure of radiofrequency catheter ablation.
Elsewhere, Conte, Schotten and Auricchio note that very little is known about the causative role of inherited cardiac channelopathies on atrial conduction abnormalities possibly leading to atrial tachyarrhythmias. The diagnostic and therapeutic management of these patients remains challenging. The authors review current knowledge on atrial electrical abnormalities affecting patients with inherited primary arrhythmia syndromes – including long and short QT syndromes, and early repolarisation syndrome – and consider the urgent need of improvement.
